EIRC Covid-19 Vaccination Plan
Plan Statement
The Erin Indoor Recreation Center (EIRC) has an obligation under provincial legislations to take all necessary precautions to protect the health, safety and wellbeing of its patrons and employees and is committed to providing excellent service to our community and to building trust and within the community.
A. Purpose
The purpose of this plan is to outline the EIRC’s requirement for patrons COVID-19 vaccination and provide patrons with direction on the requirement to provide proof of COVID-19 vaccination status or an exemption to enter the facility.
The COVID-19 pandemic has been an evolving public health issue in Ontario. The EIRC continues to monitor guidance from all applicable public health authorities and reserves the right to revise this plan in accordance with the governing bodies legislation and or mandates.
B. Definitions
“Fully Vaccinated”: In Ontario, an individual is considered fully vaccinated if they have received:

The full series of a COVID-19 vaccine authorized by Health Canada, or any combination of such vaccines;

One or two doses of a COVID-19 vaccine not authorized by Health Canada; followed by one dose of a COVID-19 mRNA vaccine authorized by Health Canada; or ,
-
Three doses of a COVID-19 vaccine not authorized by Health Canada; and,
-
They received their final dose of the COVID-19 vaccine at least 14 days ago.

“Proof of Vaccination”: Documentation issued by the Ontario Ministry of Health, other province or territory or international equivalent indicating the patrons COVID-19 vaccination status. Proof of vaccination is available at: https://covid19.ontariohealth.ca/
C. Entrance to Facility
Patrons entering the facility must comply with Sections 1, 2 or 3 and will be required to proof of vaccination along with identification or exemption letter.
Section 1. Fully Vaccinated Patron Requirements

Each patron, 18 years of age and older, who enters solely for the purpose of actively participating in an organized sport;

Each Coach, trainer, instructor, organizer, coordinator and official of the team or organization
-
Each patron, 12 years of age and older, who enters for any other reason (e.g., as a spectator)

Each patron, 12 years of age or older, who enters the facility for a birthday party
Section 2. Patron Exemptions

Youth players under the age of 18 engaging in the activities on the field

Spectating children under the age of 12, if our employee is unsure of the child’s age they can request a verification from a parent or guardian.
Section 3. Medical Exemption
Patrons with a medical exemption are required to present identification and a written document stating the individual is exempt for a medical reason to the business or organization.
To review proof of a medical reason for not being vaccinated against COVID-19, must include:
• The name of the person in the written documentation matches the identification provided.
• The physician’s or registered nurse in the extended class’s information is complete by including:

Name and contact information of the physician or registered nurse in the extended class;
-
Logo or letterhead identifying the physician or registered nurse in the extended class;
-
Statement that there is a medical reason for the individual’s exemption from being fully vaccinated against COVID-19; and
-
Any effective time-period for the medical reason which includes the date the patron is seeking access to the business or organization.
Section 4. Proof of Vaccination
To enter the facility, you must provide:

proof of vaccination (your vaccine receipt, demonstrating you are fully vaccinated and the final dose was taken at least 14 days ago)
You can provide either a paper or digital copy of your vaccination receipt that includes your:

name
-
date of vaccination
-
brand of vaccine at the time of vaccination
You can use the receipt you were given at your second dose or the QR code from the Ontario Provincial Government.
You cannot alter the appearance of your receipt in any way.
Section 5. Proof of Identification
You must prove your identity by showing identification that includes both your name and date of birth.
A photo identification is not required.
To confirm your identity, you may use your:

birth certificate

driver’s licence
-
government (Ontario or other) issued identification card

passport
-
citizenship card
-
permanent resident (PR) card
-
Indian Status Card or Indigenous Membership Card
Section 6. Compliance
Patrons in non-compliance with this plan will be denied entry to the facility until such measures are taken to be in compliance with this plan. EIRC employees reserve the rite to refuse entry to any non-compliant patron or participants
D. RESPONSIBILITIES
EIRC Executive

Create the COVID-19 Vaccination Plan for the EIRC
-
Follow all Federal, Provincial and or local Public Health mandates or directives regarding the vaccination of patrons in relation to COVID-19 in an indoor recreational facility.

Provide employees with access to information on COVID-19 health and safety precautions and mitigate COVID-19 risk of transmission within the facility
-
Always maintain confidentiality.
Employees
-
Understand and implementation of this plan, including providing information required by this plan to patrons upon request.
-
Follow all Federal, Provincial and or local Public Health mandates or directives regarding the vaccination of patrons in relation to COVID-19 in an indoor recreational facility.

Follow all EIRC Employee COVID-19 protocols.
-
Report any verbal harassment or physical abuse by patrons or participants.
-
Always maintain confidentiality.

Role model a respectful environment in interactions with others as it relates to vaccination.
Patrons
-
Understand and compliance with the EIRC Vaccination plan, including providing the required information by this plan to the EIRC employees upon request.
